A hard game for me in real time to play is Super Mario Bros. 2 (both versions). Tell me what games you had a hard time getting through!
If I could have a tool-assisted real life, I'd... Being a novice, I'd probably load the wrong state, have the IRS AI bankrupt me, and eventually make me want to kill myself and redo 11 years of hard work.

Are you asking for the hardest games ever or games we "had a hard time getting through"? As for the latter, I had some trouble getting the best ending in Cave Story. The final level is like an endurance run, with enemy and falling-rock patterns to memorize.

Cave Story best ending is not hard at all if you know what you're getting yourself into and come prepared. Can't think which one the hardest game I played is right now. Probably something that's not very hard at all either.

Silver Surfer (NES) - Bullet hell, nuff said. Keitai Denjuu Telefang (GBx) - The enemies, especially the ones in the first game, are overpowered. Which means battles are mostly luck-based. Yuu Yuu Hakusho: Makai no Tobira (GB) - Bosses are overpowered without tools/grinding. Syobon Action (Windows) - Very unfair platformer. Winter Games (NES) - I got no clue how to get pass a certain minigame. Where in Time Is Carmen Sandiego? (NES) - I fall asleep before even completing much of the game. Ao Oni (Windows) - Without tools, I'm too disturbed to play for long. Any MMORPG's that require powerleveling - Too boring, I usually just bot all day instead.

There were a bunch I played when I was young that I never beat (without cheats) like Battletoads (NES) Solstice (NES) TMNT (NES) Mega Man 3 (NES) (I beat that when I was older no problem though). Fester's Quest (NES) Rush 'n Attack (NES) Little Nemo the Dream Master (NES) Some games that were a good challenge that I still beat would be TMNT 2: The Arcade Game (NES) Dr. Mario (NES) (now I could do this in my sleep though) Blast Corps (N64) (getting all platinum) Super Meat Boy (PC) Solar Jetman (NES) Contra (NES)

I've never beat any Castlevania NES without rewinds, same with Dracula X and the Mega Man classics, lol. And a good challenge was Mega Man X6 and... Mega Man X7 <-- DON'T PLAY IT.

Many, many, many of the 80's games were extremely hard (which is why the whole trope of "Nintendo Hard" exists in the first place.) Thus there's nothing notable in listing a myriad of extraordinarily difficult games from those times. Nowadays some indie games are "Nintendo Hard" on purpose. It's their main shtick. They are difficult purely for the sake of being difficult, and little else, and the (intended) challenge is to overcome the ridiculous difficulty. These games seldom have anything else interesting in them (and even the challenge itself usually becomes boring really quick.) A more interesting topic would be games that are really hard, but also really good otherwise. Something that a casual gamer could not stand for 10 minutes, but a HC gamer gets hooked. Not because of the difficulty, but because of the game just being so damn good. It's not easy to integrate great difficulty into a game in a way that works, but it's doable with enough talent. One game comes immediately to mind: Dark Souls. This is precisely the game that's so difficult that a casual gamer will not play it for more than ten minutes, yet a HC gamer can get completely hooked. You are going to die, a lot, and dying is not just a minor issue (because you can lose everything you have worked so hard to obtain for the last half an hour of play) and can become really frustrating, but the game is just so well done that it's really rewarding when you finally make progress. To be honest, though, after something like 40 hours of gameplay it got so frustrating that I stopped playing, especially after it dawned on me that I had been wasting a significant powerup (which appears in the entire game half a dozen times, as is gone forever when you use it) by using it in the wrong way, only because you can, and because its description is misleading. Because I didn't get even a single such powerup, the game is now even more difficult than it already was.

Many people can beat battletoads and ghosts'n goblins. What I haven't seen is someone beat Holy Diver (NES). The last level is waaaay too cheap.

I'm not sure about hardest, but games like Dustforce and N and N+ get incredibly precision demanding on the later levels. You could also try Iji on Ultimortal difficulty, Hero Core on Reallyjoel'sdad difficulty (it's doable TAS, is it doable real-time?) and Super Princess Pitch on Reallyjoel'smom difficult (no clue about this one)
