For 32X it works. You just need to configure your bios files in the Misc/Bios menu (you need 68000 bios , Master SH2 bios and Slave SH2 bios. Don't ask where to get them) I'm not sure about SegaCD and SegaCD+32X games. Check the official Gens forum : http://gens.consolemul.com/cgi-bin/ikonboard/ikonboard.cgi

If there is a way to get Sega CD games to work with this, expect me to do something with the Eternal Champions sequel. MAN are the combos in that game broken... -Josh

Even if Sega CD games end up working, I doubt you'll have much success in rerecording them without desyncing, the way I understand rerecording to be implemented at least.

I think it will be better to have a VBA with rerecording first. Or, how about a N64 plugin that let you rerecord. Like modifying existing joypad plugin.

To play what games? I mean, there are only like 15 games for the N64 and that's even if you count stuff like Jet Force Gemini.

Hmm.. I seem to count 393 different N64 games.... this doesn't include region variations and such though.

Well, let's actually see here: Mario, Starfox, Zelda, Zelda, Blast Corps, Goldeneye, Perfect Dark, Yoshi's Story(shhh), DK64(shhh x 2), ummm, Mischief Makers... yeah, I'm running out of ideas.

Add those to Goemon, Get Force Gemini, Smash Brothers, Mario Kart, Kirby, both Castlevanias, Rockman Dash, Puyo Puyo Sun, and Ogre Battle, and those are all the games that I'll admit exist. And that makes 20, but it includes less than stellar games (Kirby 64, both Castlevanias, Get Force Gemini) and games that have been or will be released on other consoles (Mario 64, both Zeldas, Yoshi's Story, Puyo Puyo Sun, and Rockman Dash).
